New patients to our office should view, print out, and fill the attached new patient forms below before arriving at our office. Having these forms allows us to proceed with your treatment more quickly and completing it on time. This will make your schedule more flexible and manageable. Patient Responsibilities
If your insurance has changed please call us immediately. Bring all medications with you to your appointment.
Make sure you get to your appointment 30 minutes early to register and have your current insurance card and I.D.
If you are unable to make your scheduled appointment, it is YOUR responsibility to call our office and reschedule. If you do not notify us within 24 hours of your scheduled appointment time you will be billed $25 for established patients, $75 for new patients and returning patients that have not been seen in our office for over 2 years and up to $150 if you miss a scheduled procedure.
If follow-up visits are necessary, the patient must obtain a referral prior to scheduling an office visit.(HMO ONLY)
Do not lose your information. Doing so will result in unnecessary time delays.
Bring all X-rays/MRI films, that pertain to your related GI symptoms.
